Home Care Coordinator
We seek a dedicated and experienced Home Care Coordinator to join our multidisciplinary team that delivers exceptional care to older adults. As part of Fizzio Clinics, you will play a pivotal role in coordinating and managing home-based healthcare services, ensuring that the highest standards of clinical governance are maintained.
As a Home Care Coordinator, you will:
- Play an instrumental role in coordinating and managing home-based healthcare services.
- Collaborate with our in-house healthcare providers, physiotherapists, remedial massage therapists, and exercise physiologists.
- Maintain the highest standards of clinical governance, reflecting our unique approach.
- Clinical Governance: Ensure that all care services meet the quality and safety standards per regulatory guidelines and the healthcare model of our organisation.
- Care Planning: Develop, implement, and evaluate individualised care plans for older adults in collaboration with healthcare professionals and stakeholders.
- Resource Allocation: Efficiently allocate healthcare resources, including staff and equipment, based on patient needs and availability.
- Team Collaboration: Work closely with healthcare providers, physiotherapists, remedial massage therapists, and exercise physiologists to ensure comprehensive care.
- Data Management: Compile and analyse patient data to ensure effective service delivery and contribute to ongoing research and quality improvement initiatives.
- Patient and Family Education: Provide educational resources and support to patients and their families to enable informed decision-making and self-management.
Skills and Competencies
Qualifications – Preferred but not Essential
- Bachelor’s degree in Healthcare Management, Nursing, or a related field.
- Minimum one (1) year of experience in healthcare coordination, preferably in aged care.
- Proficient in electronic health record systems and data analysis tools.
- Familiarity with aged care and healthcare systems in Australia.
- Research experience in chronic diseases and older adult care.
- Excellent communication and interpersonal skills.
- Ability to operate within the scope of clinical governance frameworks.
- A self-starter, comfortable working from home the majority of the time.
Compensation and Benefits
- Competitive Salary: A base salary range of $70,000 – $80,000, commensurate with experience and qualifications.

About Fizzio Clinics
Fizzio Clinics started in November 2011 with one clinic in Yeronga. We have grown to seven clinics based out of luxury retirement villages, with three more currently under development. All clinics have state-of-the-art fitness centres, treatment rooms and recreation facilities. In addition to in-clinic delivery, Fizzio Clinics provides community physiotherapy in-home and in residential aged care facilities.
Fizzio Clinics is a clinical training partner with the University of Queensland. We have multiple students across the clinics for 45 weeks of the year, adding additional support for running programs and clinics.
