The "Initial Aged Care Assessment Request" is a document utilized by Fizzio Clinics for both new and returning clients, as well as their families, to request private physiotherapy services within a residential aged care setting.
Expected Outcomes of the Assessment
Informed financial consent at Fizzio Clinics is a critical element of our care delivery process, ensuring that clients and their families fully know the costs associated with the physiotherapy services provided in residential aged care facilities.
This includes clear communication of fees for initial consultations, subsequent treatment sessions, and the duration and frequency of treatment cycles. Clients are informed that standard treatments are typically 30 minutes, with variations depending on the client's condition on the day.
Additionally, our cancellation policy upholds a firm commitment to respecting the time and resources of our clinicians. We offer an email and SMS confirmation and reminder system to facilitate appointment management.
Cancellations with more than 24 hours notice do not incur a fee. However, appointments cancelled within 24 hours may attract a cancellation fee, particularly in cases of no-shows or late cancellations.
Medicare and DVA do not cover fees for missed appointments, and such charges become the client's responsibility.
We understand that emergencies can arise, and we apply discretion in these instances. However, repeated failures to attend scheduled appointments without proper notice may require pre-payment for future services to ensure clinic efficiency and availability.
